Eligibility for grant form

If you are uncertain as to whether or not your organization or project may be eligible for a grant from The Jewish Fund, we encourage you to complete the following questionnaire. If the answer is “yes” to all three questions, it is likely that the organization and project may be eligible. We will contact you soon to discuss the project and the application process in further detail.

Is the organization a non-profit qualified as tax exempt under the Internal Revenue Code?	Yes No

Is the primary service to be delivered intended to address any of the following:	health of vulnerable populations in Detroit and the surrounding areas health and social welfare needs in the Jewish community a service of the Detroit Medical Center or an affiliate organization efforts to further positive relationships between the Jewish community and the Detroit community
